One of the testbeds under the NHS testbeds programme, facilitating the Digital Care Home Project which enables care home staff to use equipment for monitoring information for residents electronically and communicate with community nursing teams or local GPs to identify the best course of action if they see early signs of patient deterioration.

Testbed purpose

The purpose is to improve the patient pathway to bring substantial benefits for patients suffering from long term health conditions such as diabetes, mental health problems, respiratory disease, hypertension and other chronic conditions. The Wave 1 test bed offered new solutions to keep patients with long term conditions well, independent and avoiding crisis points which often resulted in hospital admission, intensive rehabilitation and a high level of social care support.
